You are correct that a form is not required regarding an open records request and an email is sufficient. However, you do need to be specific with dates of events you are talking about and how you wish to review/receive this information, thus the need for the form. I am assuming you are referring to the death of Eurie Martin on Deepstep Road. If so, we have no internal interviews with the three Deputies involved. The Eastman Office of the Georgia Bureau of Investigation handled all the case immediately following the incident and would have anything related to said case.
On your request of Thomas Farthing, I will gather the releasable information and get it to you in a timely manner.
Now I will ORR the GBI for the Martin files.
